Our office hours are Monday through Friday from 8:00 am to 5:00 pm.
Your first visit
For your first visit, please bring the following items:
- Insurance card
- Referral from your care physician (if needed)
- Results of diagnostics testing (MRI's, CAT scans, etc.)
- List of prescriptions and medical conditions
- Medical records
